TVPC’s core service is providing Franchised PCPs: MD/DO/Nurse Practitioners to the most vulnerable of patients: seniors, (65+) chronically ill, veterans and those with minimal access to health care, (noun) at their homes and residences. The goal is to minimize trips to brick and mortar offices while, simultaneously, increasing healthcare (verb) services at the places of residence.

Additionally*, we partner with other organizations, spearheaded by our PCPs, that brings comprehensive, team-based care to patients at home/residence:

  • Telemedicine/Telehealth: The Village Prime Care delivers Telemedicine/telehealth services, which is the remote provision of care by electronic means e.g. computer, (primary) and/or other telephonic means. Telemedicine is the service provided by a physician, (MD/DO) or an Advance Nurse Practitioner (NP) or Physician's Assistants (PA). Telehealth is the delivery of services provided by a Nurse, Pharmacist, Physical Therapist, Psychologist (MS) or any other licensed healthcare professional. Either Telemedicine and/or telehealth are used for Primary Care followup visits and referrals, to provide additional health care services such as Mental Health Care.
  • Home Health Care: This is The Village Prime Care's (TVPC) provision of care, through one of our local partners, of services that are trained support to those at home/residence e.g. as skilled nursing care, personal care, homemaker and companion services, physical therapy and medical social services.
  • Mental Health: TVPC's mental health is provided by our mental health partners to give people with behavioral health needs the right care at the right time.
  • 24/7 Nursing: this is the availability of the services for remote nursing services, by telephone. Should a patient have health concerns at anytime, they or their caretaker can call to speak with a nurse regarding their health concerns
  • Care Coordinator: TVPC provides a Care Coordinator who assist the Primary Care Provider, the patient and/or the caretaker with coordinating any of TVPC's services for appointments, followups, referrals, etc. This is to maximize the quality of care, and customer service to achieve our mission, to deliver a superior product and service to every patient.
  • Dental Hygiene: Dental Hygienists are sent to the homes/residences of the Franchisee's patients by referral for the examination and care of the mouth and teeth for their health. Good dental hygiene is imperative as it directly affects the health status of our patients.
  • Pharmacy: TVPC intends to partner with companies such as PillPack® where they: Sort and Deliver your medications, prepacked, so they’re already to take with no confusion; supply general pharmacy items e.g. creams, inhalers, and testing supplies; provide automatic refills, and 24/7 Support. No more waiting for you or your caretaker to have to remember pick the medications up from the pharmacy.
  • Remote patient monitoring: Remote patient monitoring (RPM, a form of telehealth) utilizes digital technologies to collect medical and other forms of health data e.g. heart rate monitoring, from individuals in one location and electronically transmit that information, securely, to health care providers in a different location for assessment and recommendations.
  • Labs and applicable diagnostics: These are those procedures that can be performed at the place of home/residence. For labs, we have clinical staff come to the home/residences to draw blood, and then take the samples to an outside company for testing. Diagnostics are any biophysical testing that can be performed at the home/residence e.g. EKG, pulmonary (lung/respiratory) testing, etc.
  • Virtual visits with others TVPC patients online: through our online Virtual Village: play cards, other games, watch movies, shop together, etc.
  • Social Services: these are services that aging in place patients or caregivers may need to help with the quality of life for the patients and their family (all vetted through TVPC), e.g. banking, finance, grocery shopping, legal, and home repair.
